Application Procedures

Thank you for considering J.U.M.P. Ministries Global Church Academy for your child’s
education. We appreciate your application and look forward to guiding you through the
admission process, which includes the completion of the following steps:

o Make an appointment to have the applicant tested before registration and tour our
facility.
o Complete online or turn in the following completed forms to the admissions office:
o Application for Enrollment Form (click here)
o Scholarship award letter (if available)
o Remit all registration and curriculum fees to the administration’s office.
o Turn in the following:
o Withdrawal form from previous school
o Updated Physical Form from the State of Florida
o Up to date Immunization Record from the State of Florida
o Copy of Official Birth Certificate
o Copy of Social Security Card
o Copy of Parent’s Identification
o Proof of residence (current bill, lease, signed closing contract)
o Proof of Legal Custody Agreement (if applicable)
o Copy of Applicants Last Report Card and Transcript from Previous School

o Authorization for release of Student Records
o JMGCA Media Release Form of photographs or video of your child
o Health Consent Form
o Transportation Waiver
o Attendance policy
o Academic Integrity Agreement
o Signed acknowledgement that you have received a copy of the JMGCA Parent
Handbook.
